Leave! This guy is a total scammer. Check local or state law with regard to him keeping your money. In the USA state law varies and I'm almost certain any money not used has to be returned to the patient. Some states look at this prepayment as selling insurance and its illegal, some require the money to be put in a escrow account. Any doctor who makes his patient sign a contract is scamming you.
