Heel Spurs Under achillies Tendon

My boyfriend has heel spurs at the Achilles tendon insertion. He has been to PT and tried for 3 weeks a foot immobilizer to no avail. I am assuming that cortisone injections would not be recommended because of the location and surgery at the site of tendon insertion also risky. What would you suggest . He is very athletic and it excellent shape. This is making him very depressed as he has not been able to play squash or even walk very much for almot 6 months.
