Big Toe Getting Larger

Due to a fall a year ago, hyperextended big toe. Eventually, the toe was fine but remained a bit stiff. Xray showed everything was fine. Gradually the toe has increased in size, getting thicker in the middle of the toe (looks really fat). Xrayed and tested again, which showed nothing. The coloration is lighter, and now it is becoming more uncomfortable in my shoes. What is happening and what should I do? Very concerned because both types of diabetes runs in my family on both sides.
