Neuropathic ulcerations

I have had the right great toe removed completely (in stages), the first whole ray. The second toe "melted down" (literally) with a diagnosis of osteo. part of the second metatarsal head of the second toe was also removed. now my remaining three toes have dislocated as the toes try to move over to take the place of my missing toes/metatarsals. I am diabetic, suffer from PAD. The opening under my second metatarsal is now almost healed. What type of shoe and orthotic will help keep my foot healed? Also on my left foot, I suffer from hammertoes which I have partially alleviated with a toe crest
