Losing Big toenail

I lost my big toenail about two years ago from wearing a pair of boots to tight. Skin at bottom of toe became red and sore and toenail came off from the botton up. After a year it grew back. Six months later the same thing happened --sore, red skin around bottom of toe, nail began to come loose at bottom. But this time no tight shoe. What is wrong? Should I remove the nail that is detached already or let it fall off by itself? There is no sign of discoloration, fungus, blood, or black or brown spots. The left side is about 1/4 attached. The rest is dettached completely. Any suggestions would be great!
