left upper arm pain

I had rotator cuff repair, a/c joint repair, bursectomy that lead to chronic staph, resulting in a recurring open draining shoulder wound. after eight surgeries (including a pectoral flap), nine years of antibiotics, I still have chronic, severe upper arm pain with weakness and reduced r.o.m. i think it is a torn bicep tendon or muscle. i have knots and bruising in the area. massage relieves pain during the massage. heat is a temp. help. ice aggravates. the pain is severe. no relief form narcotics, muscle relaxants, antiinflammatories. the dr. keep insisting bursitis d/t shoulder injury. need a shoulder replacement, can't be done d/t hx. of infection. need a fresh opinion to give ideas to take to a dr.
